Kaiser Permanente Clinical Pharmacist Specialist - Springfield Medical Center in Springfield, Virginia

Develops, implements, and evaluates regional clinical pharmacy programs for patients, providers, and pharmacists. Maintains a clinical pharmacy practice to impact patients at high risk for drug-related problems in area of expertise. Educates providers, pharmacists and other health care professionals re: pharmaceutical care. Makes members/patients and their needs a primary focus of one's actions; develops and sustains productive member/patient relationships. Maintains knowledge of and assures departmental compliance with Kaiser Permanente's Principles of Responsibility and policies and procedures, and applicable regulatory requirements and accreditation standards.

Essential Responsibilities:

  • Develops and maintains a clinical pharmacy practice that includes:

  • Drug therapy consultations for members and healthcare providers regarding Specialty drug therapies, and other therapies requiring close clinical monitoring and management.

  • Participation in drug use initiatives and strategies to promote achieving targeted disease state goals and cost-effective prescribing especially as they relate to the patient population taking Specialty medications.

  • Provides continuing education and staff development in-services for pharmacists, physicians, and other health care providers especially in specialty drug therapies and protocols.

  • Provides drug information to health care providers caring for patients as well as members and families of patients taking specialty medications

  • Critically reviews medical literature to provide updates on drug therapy. Documents all clinical interventions and drug information requests from appropriate providers.

  • Demonstrated continued competency in specialty area of pharmacy practice.

  • Provides guidance and expertise when needed to specialists regarding medications and medication delivery practices.

  • Collaborates with other staff as necessary in development/revision of any policies and procedures related to appropriate delivery of specialty medications to patients.

  • Works with all areas of the Kaiser Mid-Atlantic Pharmacy and other departments, as needed, to develop appropriate ways to enhance and augment delivery of pharmaceutical care for patients taking Specialty medications. This includes, but is not limited to Clinical Pharmacy Services, Operations Pharmacy Services, Pharmacy Infusion Services, Ambulatory Surgery Center staff, and Critical Decision Unit staff.

  • Provides guidance and expertise when needed to specialists regarding medications and medication delivery practices.

  • Collaborates with other staff as necessary in development/revision of any policies and procedures related to appropriate delivery of specialty medications to patients

  • Precepts pharmacy students and residents

  • Develops and participates in research activities

Basic Qualifications:

Experience

  • Minimum completion of one (1) year residency related to practice area, or two years of equivalent clinical practice experience required

Education

  • Bachelors, Masters, or Doctoral (PharmD) degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE)

License, Certification, Registration

  • A current, valid and unrestricted Virginia Pharmacy license required. Additional licenses in District of Columbia Pharmacy and Maryland will be required within three (3) months after hire

  • Board certification recognized by the Board of Pharmaceutical Specialties required (BCPS or board certification relevant to clinical practice): within three years of employment if previous experience includes PGY1 residency or a fellowship; upon hire if previous experience does not include PGY1 residency or a fellowship

Preferred Qualifications:

  • Managed care experience in out-patient setting preferred

  • Specialty medication management experience preferred

  • Anticoagulation management experienced preferred

  • PharmD Degree preferred

  • Completion of a PGY1 residency or a fellowship program accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy preferred
