Kaiser Permanente Activity Coordinator Subaccute - Grade 5 (Temporary) in Manteca, California

Under supervision and direction, provides programs for the various needs of residents in accordance with multi-disciplinary care plan and physicians orders. In doing so, observes residents, collects and records pertinent resident data, provides therapeutic activities, works closely with nursing staff to developing resident care skills.

Essential Functions:

  • Develops program for and implements activities which consist of individual and small and large group activities designed to meet the needs and interests of each residents. Includes, but are not limited to:

  • social activities

  • indoor and outdoor activities, which may include walks

  • activities away from the facility

  • religious programs

  • creative activities

  • educational activities

  • exercise activities

  • Involves residents in planning and implementation of the activity program

  • Supervises the activity program

  • Assists with planning of in-service training of the staff of the facility least annually

  • Coordinates the activity schedule with other resident's services

  • Checks daily with charge nurse for residents who are not physically able to participate in activities

  • Requests and maintains activity program equipment and supplies

  • Develops and maintains activities for residents unable to leave their rooms

  • Develops and maintains contracts with community agencies and organizations

  • Maintains progress notes specific to the resident's plan which are recorded at least quarterly, and more frequently if needed, in the resident's health record

  • Maintains a current record of the type and frequency of activities provided and the names of residents participating in each activity

  • Assists with providing seven days per week activities

  • Post the activity schedule conspicuously, for the information of residents and staff

  • Where appropriate, may recruit, train, and supervise a volunteer program to assist with and augment the services of the activity program

  • Interviews and assesses each resident within four days of admission to determine his/her background and programming needs

  • Develops an appropriate individualized care plan based on those needs

  • When appropriate, designs and puts large wall and individual room calendars of scheduled activities, for the information of residents and staff

  • Assist with maintenance of equipment and supplies

  • Assist with development and maintenance of community agencies and organizations

  • When appropriate conducts bi-monthly resident council/resident survey to collect and evaluate resident's feedback

  • Directs any compliments, concerns, or questions to appropriate department

  • When appropriate, attends and contributes to resident care conference and other staff meetings

  • Inputs appropriate patient information, assessments, care plans into Computerized Patient Care Documentation System within appropriate time frame

  • Initiates and completes MDS within designated time period

Basic Qualifications:

  • Two years of experience in a social or recreational program within the past five years, one year of which was full-time in a resident activities program in health care setting or satisfactorily completed or currently enrolled in at least 96 hours of training in a course designed specifically for this position and approved by the Department of Licensing for SNF's.

  • Interpersonal skills at a level to effectively communicate with patients, volunteers, community agencies, and other hospital employees.

  • Capable of generating enthusiasm for activities with both patients and staff.

  • Reading and writing at a level normally required through completion of a high school education.
