In collaboration with physicians and other members of the care team, our clinical pharmacists provide a wide array of clinical pharmacy services, ranging from primary care to targeted high-risk, high-cost specialty care. They care for diverse patient populations across all lines of business (Commercial, Medicare, Medicaid, and Exchange). The majority of patient care is provided virtually via telephone and secure messaging, however, decentralized clinical pharmacists also have the opportunity to work with patients face-to-face in the clinic.

Sample patient care activities include:

Collaborative Drug Therapy Agreements (CDTA) in various therapeutic areas

Primary Care CDM (chronic disease management): asthma, diabetes, ASCVD, and hypertension

Specialty CDM opportunities: Hepatitis C, HIV/PrEP, MS

Medicare Medication Therapy Management (MTM) Program

Medication consults to primary care and specialty care teams – dosing, drug selection, interactions, monitoring, adverse effects, safety

Hospital discharge medication reconciliation

Medication Use Management (MUM) – initiatives to improve affordability and/or safety

Quality – initiatives to improve patient care and organizational performance on HEDIS and CMS 5-Star measures Qualifications:

Education Minimum Education Requirements Doctorate Degree PharmD Completion of PGY1 residency (or equivalent) Preferred Education Doctorate Degree PharmD Completion of PGY1 and PGY2 residency (or equivalent) Knowledge/Skills Minimum Knowledge/Skills Requirements

Must have excellent verbal (including presentations) and written communication skills. Strong leadership skills and ability to lead teams. Ability to problem solve and innovate. Must have advanced skills with Microsoft suite - including Microsoft Word, Excel, and PowerPoint. Experience in educating physicians. Must be able to evaluate data, interpret findings, and clearly communicate findings and recommendations to target audiences, including, but not limited to physicians, nurses, patients. Must have adequate current knowledge of major medication therapy, drug-drug interactions and disease states. Must have the ability to work effectively in a team environment. Must have the ability to prioritize workload effectively. Must be able to work quickly, with accuracy and efficiently. Must have the ability to use technology, available resources and tools efficiently and effectively. Able to seek out additional tools and resources as needed. Must have excellent customer service skills. Preferred Knowledge/Skills

Fluent with MS Work, Excel, and PowerPoint Experience Minimum Experience Requirements 3 years of clinical pharmacy experience with high risk/high cost populations (or equivalent). Experienced clinical leader in working with interdisciplinary teams. Must have strong medication evaluation skills and ability to apply to patient care. Preferred Experience 3 years of clinical pharmacy experience in primary care or specialty care, preferably in an ambulatory care environment and as part of a multidisciplinary/interdisciplinary team . License/Certifications Minimum License/Certifications Requirements At time of hire Washington state pharmacist license and BPS certification

If not already BPS certified, individual will be expected to become BPS certified within 1-year of BPS eligibility as condition of employment.
